Payroll Deduction Authorization

Payroll Deduction Authorization Form

The Payroll Deduction Authorization Form authorizes the deduction of mandatory working dues and the voluntary $0.04 per hour COPE-PAC contribution from the apprentice’s wages. Employers should ensure they have a Payroll Deduction Authorization Form on file for all apprentices. We recommend including this authorization as part of the standard new-hire paperwork for apprentices.

As a courtesy, the Boston JATC will provide a completed and apprentice-signed Payroll Deduction Authorization Form to an employer for the apprentice’s first-ever job assignment only. The apprentice completes and signs this form as part of the JATC’s initiation process.

If an apprentice is subsequently laid off and assigned to a different employer, the new employer is responsible for having the apprentice complete and sign a new Payroll Deduction Authorization Form as part of their new-hire paperwork. The JATC does not provide a pre-signed form for subsequent employer assignments.
